This article explains how to add a form item using the helferlein form module for DNN.  To add a form item follow these instructions:

1. On the module options select Add form item
2. Complete the form configuration as follows:
   1. The "Type" field specifies the type of filed used and also the validation used.  For example a field type of Date requires a date to be entered
   2. The "Label" field is the name of the field that will be displayed to the entry field
   3. The "Help text" is the text displayed if the user puts the cursor over the help question mark
   4. The "Width field specifies the width of the entry box, eg 100px
   5. The "Lines" field specifies the number of lines in the box, eg when specifiying a type of Multiple-Lines
   6. The "Repeat columns" field specifies how many columns to display the multiple select options
   7. The "Link target" specifies the action required for the window when the link is selected
   8. The "Value(s)" field specifies the default values
   9. The "Required" specifies if the field must be completed before the form is submitted
3. Press "Update"

Once the form is created the order of the fields can be changed by selecting edit for the field.  You will then be provided with options to move the field.
